This is a preliminary application for your child's
enrollment in our school and does not guarantee
placement. Please fill out the form completely. Type
"N/A" or 'Not Applicable" for those questions that do
not apply. Then hit the "submit" button at the bottom of
the page.
We will notify you within 24 hours to confirm receipt of
the form and your child's position on the waiting list
(if we are already full for the school year you are
requesting, at the time you submit your pre-enrollment).
